Synchronize the execution ledger's written file descriptor before reporting append success or publishing a newly created ledger. Surface synchronization failure without retrying or removing uncertain appended bytes.

Add storage-fault, short-write and fresh-process termination regressions to the local suites. File synchronization does not guarantee directory metadata durability or replace shared locking and recovery.
